How MSC-Derived Exosomes Are Sourced and Released
Exosome preparations used clinically are isolated from the culture medium of mesenchymal stem cells, most often umbilical cord-derived cells taken from screened, consented donations after healthy full-term births. The donor pathway matters, because everything downstream inherits its quality.
A provider should be able to state where the parent cells came from, which laboratory expanded them, how the conditioned medium was collected, and by what method the vesicles were isolated. If any link in that chain is described vaguely, treat the vagueness as the answer.
Lot Testing and Potency Documentation
Because an exosome product contains no living cells, quality is judged entirely on the release testing of the batch. Sterility and endotoxin testing are the minimum. Beyond that, particle counting and size distribution, surface marker characterisation and protein content give a picture of what is actually in the vial.
Potency claims deserve particular scepticism. There is no universally accepted potency assay for exosomes, so a clinic quoting a single impressive number without naming the method is marketing rather than reporting. Ask for the certificate of analysis for the lot you will receive.
IV Versus Localised Delivery
Route is a clinical decision, not a preference. Intravenous delivery is used when the intended effect is systemic and the target tissue is diffuse. Localised delivery — injection into or around a defined structure, or topical application immediately after controlled skin injury — is used when the target is anatomically specific, as in a single joint or a treated facial area.
In practice, many programmes combine an assessment of the underlying condition with the route most plausible for it, and document the reasoning. Who Is and Is Not a Candidate
Candidacy is decided after a physician reviews your history, current medications, and recent laboratory or imaging results. The people most often considered are those with a defined, stable problem, realistic goals and no contraindication to a biological preparation.
Deferral or refusal is normal. Active untreated malignancy, uncontrolled infection, pregnancy and unstable cardiac or renal disease without specialist clearance are standard reasons to say no. A clinic that accepts every enquiry is not applying criteria at all.
What a Course Involves on the Ground
A typical visit runs as a sequence rather than a single appointment: remote record review before travel, in-person consultation and baseline testing on arrival, treatment on a scheduled day, then an observation period and a review appointment before departure.
Written documentation should travel home with you — what was given, from which lot, on what date, plus aftercare instructions your own doctor can read. That paperwork is also what makes any later assessment of response meaningful.
Questions to Put to the Provider
Five questions separate a medical programme from a wellness package quickly: Which laboratory produced this preparation? What release testing was performed on this lot? Why is this route appropriate for my condition? What would make you decline to treat me? What will you give me in writing afterwards?
