    Lot Testing and Potency Documentation

    Because an exosome product contains no living cells, quality is judged entirely on the release testing of the batch. Sterility and endotoxin testing are the minimum. Beyond that, particle counting and size distribution, surface marker characterisation and protein content give a picture of what is actually in the vial.

    Potency claims deserve particular scepticism. There is no universally accepted potency assay for exosomes, so a clinic quoting a single impressive number without naming the method is marketing rather than reporting. Ask for the certificate of analysis for the lot you will receive.

    Route is a clinical decision, not a preference. Intravenous delivery is used when the intended effect is systemic and the target tissue is diffuse. Localised delivery — injection into or around a defined structure, or topical application immediately after controlled skin injury — is used when the target is anatomically specific, as in a single joint or a treated facial area.

    In practice, many programmes combine an assessment of the underlying condition with the route most plausible for it, and document the reasoning.     Who Is and Is Not a Candidate

    Candidacy is decided after a physician reviews your history, current medications, and recent laboratory or imaging results. The people most often considered are those with a defined, stable problem, realistic goals and no contraindication to a biological preparation.

    Deferral or refusal is normal. Active untreated malignancy, uncontrolled infection, pregnancy and unstable cardiac or renal disease without specialist clearance are standard reasons to say no. A clinic that accepts every enquiry is not applying criteria at all.

    Written documentation should travel home with you — what was given, from which lot, on what date, plus aftercare instructions your own doctor can read. That paperwork is also what makes any later assessment of response meaningful.
